However, specialists in treating malignant mesothelioma at the leading cancer centers often have better statistics.&lt;br /&gt;&lt;br /&gt;For instance, the five-year survival rate has approached 40% for selected patients of Dr. David Sugarbaker at Brigham and Women’s Center in Boston. To qualify for Dr. Sugarbaker’s treatment you must meet certain criteria. One of them is being in the early stages of the disease, so time is of the essence. Sugarbaker that appeared in the The Journal of Thoracic and Cardiovascular Surgery, January 1999 (12 pages in Adobe PDF format).&lt;br /&gt;&lt;br /&gt;TRADITIONAL CARE&lt;br /&gt;&lt;br /&gt;There are three traditional kinds of treatment for patients with malignant mesothelioma:&lt;br /&gt;&lt;br /&gt;    * Surgery (taking out the cancer)&lt;br /&gt;    * Chemotherapy (using drugs to fight the cancer)&lt;br /&gt;    * Radiation Therapy (using high-dose x-rays or other high-energy rays to kill cancer cells)&lt;br /&gt;&lt;br /&gt;Often two or more of these are combined in the course of treatment.&lt;br /&gt;&lt;br /&gt;MESOMARK BLOOD TEST&lt;br /&gt;&lt;br /&gt;NEW! In January 2007, the Food and Drug Administration (FDA) approved the MESOMARK assay to help monitor response to treatment in epithelial and bi-phasic malignant mesothelioma patients. A specific protein, or biomarker, called Soluble Mesothelin-Related Peptide (SMRP), may be released into the blood by mesothelioma cancer cells. By measuring the amount of SMRP in a blood sample, doctors may be able to better monitor a patient's progress. Based on the limited amount of data currently available, use of this test may be beneficial, but effectiveness has not been determined at this time. The MESOMARK blood test has NOT yet been approved for the early diagnosis of mesothelioma.&lt;br /&gt;&lt;br /&gt;This test has been approved as a Humanitarian Use Device (HUD), meaning that physicians must follow certain procedures to qualify their patients for testing. Once the physician is certified, informational brochures will be sent to be distributed to each applicable patient.&lt;br /&gt;&lt;br /&gt;Those wishing to take part in MESOMARK testing will be asked to provide one or more samples of blood. The blood samples will then be sent to a national reference laboratory for testing. In conjunction with other clinical and laboratory data obtained by your doctor, decisions regarding your treatment and care may be simplified. You may discontinue testing at any time.&lt;br /&gt;&lt;br /&gt;The costs associated with the MESOMARK blood test may not be covered under health insurance, therefore, you may be required to pay all or part of the costs out of pocket. It is recommended that you check with your insurance carrier to determine whether coverage is available under your policy.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;SURGERY&lt;br /&gt;&lt;br /&gt;  Diagnostic Procedures&lt;br /&gt;&lt;br /&gt;    As previously mentioned in the "Symptoms" section of this website, a diagnosis of mesothelioma from fluid is many times inconclusive. Given this fact, diagnostic surgery becomes a necessary next step in confirming and staging mesothelioma.&lt;br /&gt;&lt;br /&gt;    Thoracoscopy enables a physician to evaluate the pleural cavity and to conduct multiple tissue biopsies under direct vision. In up to 98% of cases, a definitive diagnosis can be obtained. Often, chemical pleurodesis aimed at relieving the accumulation of fluid in the intrapleural space, can be accomplished during the same procedure. It is also possible to gauge the extent of the tumor, and make a determination of surgical resectability. While less invasive than an open biopsy, it can only be performed on patients where tumor has not obliterated the pleural space.&lt;br /&gt;&lt;br /&gt;    VATS, or video-assisted thoracic surgery is an alternative to thoracoscopy, although because of its more invasive nature, concerns of tumor seeding increase. By utilizing small incisions, the physician can view the pleural space with the assistance of a camera, and obtain sufficient tissue samples for analysis by a pathologist. Extent of the tumor (i.e., pleural involvement, chest wall invasion) may also determined, and recommendation as to the type of debulking procedure necessary can be made at this time.&lt;br /&gt;&lt;br /&gt;    Mediastinoscopy is sometimes used as an aid in staging extent of disease when enlarged nodes are seen using imaging techniques.&lt;br /&gt;&lt;br /&gt;    Laproscopy is used in mesothelioma patients in cases where imaging techniques suggest possible invasion of the tumor through the diaphragm. This information can be important in evaluating a patient for potential pleurectomy or extrapleural pneumonectomy.&lt;br /&gt;&lt;br /&gt;   Palliative Procedures&lt;br /&gt;&lt;br /&gt;    Palliative surgical procedures are those which treat a symptom of mesothelioma, without aggressively treating the disease itself.&lt;br /&gt;&lt;br /&gt;    Chest Tube Drainage and Pleurodesis is considered the most common of palliative treatments. Fluid build-up, or pleural effusion, is most often the first symptom which will prompt mesothelioma patients to seek medical attention. Once this effusion has occurred, it is many times persistent, returning rapidly after initial thoracentesis (draining of the fluid). In order to eliminate this problem, the pleural space must be closed. This is accomplished by use of a talc slurry or other sclerosing agent which produces an adhesion.&lt;br /&gt;&lt;br /&gt;    Thoracoscopy and Pleurodesis is done in conjunction with VATS using a powdered form of talc versus talc slurry. Both this and chest tube drainage and pleurodesis will be only effective if there is no tumor encasing the lung which restricts its expansion.&lt;br /&gt;&lt;br /&gt;    Pleuroperitoneal Shunt plays a limited role in palliation for several reasons. It involves placement of a catheter run under the skin from the pleural to the peritoneal cavity. Obstruction of the catheter and possible seeding of the tumor into the abdominal cavity may be concerns.&lt;br /&gt;&lt;br /&gt;    Pleurectomy, used as a palliative procedure, may be performed where more extensive surgery is not an option. In these cases, it is understood that all visible or gross tumor will not be removed. It is considered the most effective means of controlling pleural effusion in cases where the lung's expansion is restricted by disease.&lt;br /&gt;&lt;br /&gt;   Potentially Curative Procedures&lt;br /&gt;&lt;br /&gt;    These procedures are performed with "curative intent". Their goal is removal of all gross disease, with the knowledge that microscopic disease will most likely remain. Adjuvant therapy (another form of treatment in addition to the primary therapy) is typically aimed at eliminating residual disease.&lt;br /&gt;    For Pleural Mesothelioma:&lt;br /&gt;&lt;br /&gt;        * Pleurectomy/Decortication is usually performed on patients with early stage disease (Stage I and selected Stage II), and attempts to remove all gross tumor. If it is found that all tumor can not be removed without removing the lung, this may be done at the same time and is called pneumonectomy.&lt;br /&gt;&lt;br /&gt;        * Extrapleural Pneumonectomy is considerably more radical than other surgical approaches, and should be carried out by surgeons with great expertise in evaluating patients and performing the procedure itself. Because in the past surgery alone has failed to effect a cure, or even to help prolong life for any extended period of time, it is currently being combined with traditional chemotherapy and/or radiation, or other new approaches such as gene therapy, immunotherapy or photodynamic therapy.&lt;br /&gt;&lt;br /&gt;          General Patient Selection Criteria for Extrapleural Pneumonectomy&lt;br /&gt;&lt;br /&gt;          Extrapleural pneumonectomy is a serious operation, and doctors experienced in this procedure choose their patients carefully. It is up to each individual surgeon to advise the patient on its feasibility and to conduct whatever tests he/she feel are necessary to optimize the patient=s chances for survival and recovery. Following is a general list of patient selection criteria. This list may not be all inclusive, and may vary according to the preference of the surgeon.&lt;br /&gt;              o Karnofsky Performance Status score of &gt;70. This score relates to what symptoms of disease the patient may be experiencing and how well they are able to conduct their daily activities. Some surgeons may require a higher performance status than others.&lt;br /&gt;              o Adequate renal (kidney) and liver function tests; no significant kidney or liver disease.&lt;br /&gt;              o Normal cardiac function per electrocardiogram and echocardiography.&lt;br /&gt;              o Adequate pulmonary function to tolerate the surgery.&lt;br /&gt;              o Disease limited to the ipsilateral hemithorax (the same side of the chest in which the mesothelioma is located) with no penetration of the diaphragm, extension to the heart or extensive involvement of the chest wall.&lt;br /&gt;              o Age of the patient is taken into consideration, but may not be as important as their overall status.&lt;br /&gt;&lt;br /&gt;          Surgeries of this nature should always be done with a complete understanding of the possible benefits and risks involved. If you are considering surgery as a treatment option, speak openly with your doctor about your concerns, and be sure all of your questions are answered to your satisfaction.&lt;br /&gt;&lt;br /&gt;    For Peritoneal Mesothelioma:&lt;br /&gt;&lt;br /&gt;        * Cytoreductive Surgery is aimed at removing all or nearly all of the gross or visible tumor in the peritoneal cavity. In order to treat any remaining cancer cells, Intra-Peritoneal Hyperthermic (heated) Chemotherapy (IPHC) is then delivered to the abdominal cavity. The type of chemotherapy drug used may vary according to the physician’s preference. This is mainly because British cuisine varies widely, depending on what part of the country you're visiting. The cuisine of London, for example, is far different from the cuisine of Yorkshire, or the cuisine of tiny, unfamiliar regions scattered across the country and virtually unknown to Americans. In my opinion, the true cuisine of the British is not what is found in the big cities, but the unknown treasures of the table that are hiding in the farmlands and countrysides and old villages across Great Britain.&lt;br /&gt;&lt;br /&gt;If you are ever wandering the British countryside, and you stop at a local pub or restaurant for breakfast, prepare yourself. The classic British breakfast is a large meal, bigger than what we're used to as Americans, and most of it tends to be fried. Fried bacon and eggs, fried bread, and fried tomatoes are standards. The true British country experience involves a breakfast heavier than your knapsack.&lt;br /&gt;&lt;br /&gt;Asking for coffee with your breakfast in the UK is just no fun. Give the tea sensation a try. British cuisine leans heavily on tea, served with milk and sugar, the latter of which is usually coarse, brown, and unrefined. Tea is served for any meal and any time in between. It's just as classically British as it sounds. Any typical British meal, whether it's breakfast, lunch or dinner, tends to consist of some form of potatoes. Especially in the countryside, the British rely heavily on potatoes, and serve them in very traditional manners. A wonderful British treat is something called a pasty. Meat, potatoes, vegetables and warm gravy are wrapped in a flaky bakery crust and sold ready to eat. Pasties are treated like take-out sandwiches or fast food, walking down the street with a paper cone or napkin wrapped around them. They keep your hands warm too!&lt;br /&gt;&lt;br /&gt;The other major staple of typical British food is, of course, fish and chips. Fish and chip shops abound in all cities in England. British fish and chips are amazingly crackly, cooked until the coating is rich brown and salty, and the meat inside tender white and flaky. Chips, or potato wedges, are served hot and crispy on the side, and generally the whole thing is smothered in as much vinegar and salt as the consumer can stand. There is something distinctly British about that malt vinegar- left on the tables at restaurants like American ketchup.&lt;br /&gt;&lt;br /&gt;The smells and flavors of traditional British cuisine are well worth experiencing. It was also commonly used in homemade biscuits, salad dressings such as ranch and as the dipping liquid for fried chicken.&lt;br /&gt;&lt;br /&gt;Today buttermilk is mass produced and has only a trace resemblance to the original form of buttermilk. In today's dairy processing facilities a lactic acid bacteria is added to non-fat milk and allowed to ferment. This modern version of buttermilk contains calcium, protein, and vitamin B2 which make it a healthier alternative to traditional buttermilk.&lt;br /&gt;&lt;br /&gt;Today's buttermilk is lower in fat than traditional buttermilk due to the lack to butter particles and the base being a non-fat milk. It is also thicker and tangier than traditional hand churned buttermilk.&lt;br /&gt;&lt;br /&gt;A homemade version of buttermilk can be easily made with a buttermilk starter. Simply heat a scant 4 cups of non-fat milk until it is slightly warm. Do not allow the milk to come to a boil. Next add � cup of store bought buttermilk. Allow the milk to stand overnight. You don’t always have five hours to prepare a gourmet meal. In fact, most of the time it’s hard to even find an hour to throw something together.&lt;br /&gt;&lt;br /&gt;Here are some fast, easy tricks, tips and techniques to save you time and energy.  &lt;br /&gt;Buy a salad mix: Those pre-made mixtures have come a long way. They’re not just lettuce anymore. Dressing, toppings such as almond slivers and dried cranberries, and more make it look like you really spent some time. Open the bag and you have salad.  Prefreeze your Chicken Breasts: It’s always good to have some chicken in the freezer. They’ll thaw in minutes and you’ll have a nice meal ready for the family.&lt;br /&gt;&lt;br /&gt;Frozen bread dough: There’s so much assortment these days. From crescent rolls to biscuits to the traditional dinner roll, they’re all available and you’re house will smell delicious while they’re baking. Who has time for yeast anyway?&lt;br /&gt;&lt;br /&gt;Frozen fruits: Will save you money and time by allowing you to have strawberries in the middle of winter.  Yum.  &lt;br /&gt;&lt;br /&gt;Shredded cheese: Most recipes call for cheese to be shredded anyway so why not buy it preshredded?    Canned stock and broth: Who really has 24-48 hours to make stock from scratch?  Not worth it at all.   Shelf pasta: It’s a really quick simple meal when you’ve got nothing else to make. Also it lasts forever, so you don’t have to worry about it going bad.  Salad dressings: Paul Newman is doing us all a favor by having his face on so many different varieties of salad dressing, plus his version won’t spoil as fast and probably costs less.  Artichoke hearts: As much as I love a good fresh artichoke the work you have to do to get that little heart is sometimes not worth it.&lt;br /&gt;&lt;br /&gt;Tomatoes: Canned tomatoes are awesome and lots of recipes call for them nowadays instead of the fresh kind. This is not surprising since so much has been blamed on coffee. Heart disease, cancer, osteoporosis, you name it. At one time or another, coffee has been made to blame. Coffee has joined the ranks of red wine and chocolate as a guilty pleasure that may actually be good for you. As much as it is a great way to get that fix in the morning, recent studies have shown that coffee has been found to be rich in antioxidants and contains hundreds of pain-relieving and anti-bacterial compounds. Although one should not drink coffee with wild abandon, it's comforting to know that maybe that cup or two in the morning may be good for you. Here is a statistic to ponder: did you know that there are over 160 million coffee drinkers in the Unites States alone? In terms of consumption, the average American consumes 8.8 lbs. of coffee per year. But consider this: the world leader in terms of consumption is Finland which comes in at nearly 30 lbs per year. That's a lot of coffee!&lt;br /&gt;&lt;br /&gt;Another statistic is that there have been well over 19,000 studies done to analyze the effects of coffee. Now, studies can often be skewed to show whatever the author wants, but here is what is generally agreed: consuming 2 to 3 cups of coffee per day is often considered more beneficial than harmful. Recent studies have shown that moderate coffee drinking can reduce the risk of colon cancer (due to it's ability to keep you regular), gallstones, cirrhosis of the liver and more, due to the fact the it is loaded with healthy antioxidants. So loaded, that studies have implied that coffee contains higher levels of antioxidants compared to other commonly consumed beverages, such as tea and fruit juice. Several studies are also being conducted on Parkinson's and diabetic Patients. Research has shown that just one cup of coffee per day can halve your risk of Parkinson's, the brain disease that causes tremors and affects movement. Caffeine may protect the brain cells typically lost to Parkinson's disease. However, it has been noted that women on hormone therapy do not seem to benefit. It is felt that Estrogen may dilute the effectiveness of caffeine. It has also been found that if a woman drinks three cups of coffee a day, she can reduce her risk of Type 2 Diabetes by twenty to thirty percent. Coffee may help promote the delivery of insulin to the tissues. Experts believe that coffee's antioxidants, such as chlorogenic acid and caffeine acid, deserve the credit. There are also on going studies that are producing favorable reports on coffee helping to fight Alzheimer's due to the caffeine stimulating the cognitive area of the brain.
